General
County IN# = the CAD number. Can be obtained of IAR dispatch or CAD Times
(If you then click IMPORT, address and times except Dispatch should autofill)
Station # - Enter 1, 2, 3 or 4, the Station where you came from
Base-site = The base-site where the call was DISPATCHED for (AKA, which station was dispatched)
Type of Service
Times
Times should pre-fill if you used the IMPORT function. You must update DISPATCHED time or enter any missing times. These can be obtained off the CAD Times sheet (See Page 9 Attachments for info)
Referring
Under OTHER TYPE: Select from dropdown most appropriate, ie Interstate Highway, Bedroom in House, kitchen in restaurant, etc.
Location, enter location.
Requester - HC911 and then caller, if that's included.
Scene Grid - This is an easy look up - an IAMRESPONDING is a 2 digit code in parathesis (45). This is the SCENE GRID.
Receiving
Select Hospital Name. If you need to search, use the magnifier and type first part of name then "%"
PRINT last name of receiving nurse
Dest. Grid is municipality of hospital you transferred too
Comment - Input bed # or location where you transferred patient.
PRESS - Calculate Geocoding Mileage to re-calculate.
Medevac Transports ***UPDATED 7/16/2019***
Disposition: Treated, Transferred to Air Medical
Receiving: HOSPITAL - Select hospital patient was transported to by Air Ambulance.
In the COMMENTS box of receiving, put the Air Ambulance utilized and where the Landing Zone was.
This will allow our PCR to be transferred to the receiving hospital, and for us to be able to get HL7 information back from the receiving hospital.

- Emergency Response - Primary Response Area - All dispatches in our primary areas, including contract areas, such as Tewksbury during the day on dual alert.
- Emergency Response - Mutual aid: Any mutual aide call outside of primary hours, or after contact hours, such as Tewksbury nights and weekends.
- Hospital/Non-Hospital Transfers for non-emergency transports (NETs), select the appropriate option.
- Station 1: Town of Clinton
- Station 2: Lebanon Borough
- Station 3:High Bridge Borough
- Station 4: Hampton Borough
- Staging: Lebanon Township (Usually).

Facility Transfer Forms - Scan and attach copy of Facility Transfer Form for all patients transported out of a facility
